Epitalon is a synthetic tetrapeptide, Ala-Glu-Asp-Gly, with free base formula C14H22N4O9, molar mass 390.35 and CAS 307297-39-8, designed in the late 1980s at the St Petersburg Institute of Bioregulation and Gerontology. Reviewing it for the FDA Pharmacy Compounding Advisory Committee in July 2026, the agency stated that epitalon and epithalamin are different substances and that none of the 26 articles submitted by the nominators discussed the use of epitalon.
Every so often a compound's entire reputation turns out to be a citation error, and this is the shelf's clearest case.

The famous longevity studies everyone cites were run on epithalamin, a crude extract of cow pineal gland. A soup, not a sequence.
The tetrapeptide was later proposed as the extract's active ingredient, which is a hypothesis, and the literature migrated the extract's results onto the synthetic as if the hypothesis were proven.
The FDA, reviewing the compound, said it plainly: they are not the same substance.

What is epitalon?
Mechanism studied, not outcome promised.
The supporting work is in vitro: human somatic cells showing telomerase activity and telomere elongation. The intermediate steps to a human effect have not been published.
The four residues were chosen from the amino acid composition of epithalamin, a bovine pineal extract, on the reasoning that a defined peptide might reproduce what the mixture did.
The telomerase story is the claim. The identity switch underneath is the problem.
The headline mechanism attached to this molecule is telomerase activation, which is exactly the kind of claim that deserves slow reading.
Telomeres cap your chromosomes, they shorten with cell division, and telomerase rebuilds them. The idea of switching that on is genuinely seductive.
The supporting work is thin, old, largely from the same circle that developed the compound, and partly on the extract rather than the peptide.
And there is a sharp edge nobody selling it mentions: cells that reactivate telomerase at will are how cancers achieve immortality. A true telomerase switch would be double-edged by definition.
Here's the honest part.
Either it does not meaningfully activate telomerase, in which case the pitch fails, or it does, in which case the safety question is enormous and unstudied. The marketing wants both halves and cannot have them.
The research so far for Epitalon Peptide
Where the evidence is thin, we say so.
One study: 75 healthy women, 40 randomised, sublingual spray at 0.5 milligrams daily for 20 days, measuring a urinary melatonin metabolite. Blinding not specified. Plus cell culture telomerase work.
The famous longevity data. 266 elderly subjects over six to eight years, and follow-ups out to fifteen years, all using the bovine pineal extract, all from one research group, never independently replicated.
FDA reviewed the compound for its July 2026 advisory committee. The nominators submitted 26 articles. FDA wrote that none of the articles submitted discussed the use of epitalon, that seven discussed epithalamin which it considers a different substance, and that its own literature search identified no clinical studies assessing the safety of epitalon in humans.
The famous studies used the extract. This is not the extract.
The file needs sorting into three piles before it means anything.
Pile one: epithalamin extract studies. Decades old, Russian, on a bovine gland soup. These are the famous longevity numbers.
Pile two: synthetic epitalon work. Much smaller, mostly cell and animal, same originating circle. Pile three: independent replication. Nearly empty.
The FDA reviewed it in 2026 terms for compounding purposes and was blunt about the identity mismatch and the evidence gap.

Epitalon Peptide backstory
How it got here.
Soviet and then Russian peptide research pursued short regulatory peptides as drugs for decades, largely separately from Western pharmaceutical development. The St Petersburg Institute of Bioregulation and Gerontology under Vladimir Khavinson was the centre of it.
The method was consistent: take an animal tissue, extract a polypeptide fraction, use it clinically, then attempt to identify a short defined peptide within it that carries the activity. Thymalin came from thymus by the same route.
Epithalamin was the pineal extract. Epitalon is the four residue peptide designed from its composition.
The clinical work stayed with the extract. The synthetic peptide became the product that travelled internationally, and it carried the extract's reputation with it.

One that FDA could identify. Ivko and colleagues in 2021 studied 75 healthy women aged 40 to 50, mostly night shift workers, randomising 40 with low melatonin to placebo or a sublingual spray at 0.5 milligrams daily for 20 days, measuring urinary 6-sulfatoxymelatonin. The publication did not specify blinding. There are zero registered clinical trials of epitalon on ClinicalTrials.gov.
FDA reviewed it for the Pharmacy Compounding Advisory Committee in July 2026 and proposed not adding epitalon free base or epitalon acetate to the 503A bulks list, citing poor physicochemical characterisation, lack of human safety data, immunogenicity concerns and no evidence of effectiveness for the proposed use. Of the 26 articles the nominators submitted, FDA wrote that none discussed the use of epitalon.
The supporting evidence is cell culture. Human somatic cells in vitro have shown telomerase activity and telomere elongation with this peptide. That is a real in vitro finding. What has not been published is evidence that it reaches relevant cells at achievable doses in a person and does the same thing there.
Because independent replication is how a finding becomes established rather than reported. The epithalamin longevity literature, spanning 2003 to 2011 and including a fifteen year follow-up, is authored by Khavinson or Korkushko, published in Russian institutional journals, and has not been reproduced by any group outside that network. That is a description of the evidence base, not an accusation about it.
It is not named on the WADA 2026 Prohibited List, but section S0 captures any pharmacological substance with no current approval by any governmental regulatory health authority for human therapeutic use. Epitalon has no such approval anywhere, so S0 applies and it is prohibited at all times.
