An eight amino acid peptide derived from cholera toxin and deliberately built not to be absorbed, so it works only inside the gut.

Larazotide is an eight amino acid peptide with the sequence GGVLVQPG, also known as larazotide acetate or AT-1001, that acts on the surface of the intestinal lining to promote tight junction assembly and oppose zonulin driven permeability. It was designed not to be absorbed into the bloodstream, has been studied orally in celiac disease through phase 2 and phase 3 trials, and is not approved by any regulator.

This compound's origin story starts with cholera, which is not a sentence you get to write often. Studying how cholera toxin loosens the gut wall taught researchers something basic: the junctions between gut cells are not welds. They are gates, actively managed, with signalling that opens and closes them. > A pathway called zonulin emerged as one of the gate-openers. How it got here.

Alba Therapeutics originated the compound and ran the phase 2 programme. It licensed to Innovate Biopharmaceuticals in March 2016, which became 9 Meters Biopharma, which went bankrupt in July 2023.

Along the way there were pivots. A four-patient open-label case series in children with multisystem inflammatory syndrome after COVID reported faster resolution of gastrointestinal symptoms against 22 retrospective historical controls.

The authors themselves wrote that a double-blind randomised placebo-controlled study would be needed to determine efficacy. It is a case series in four children, not evidence of effect.

After the phase 3 futility call, nothing survived.

Across earlier trials different low doses won. Reviewers have called the pattern curious. An inconsistent, inverted dose-response is a reason to be cautious about whether an effect is real.
